The successful candidate will be responsible for providing support to clients by assessing their needs, developing individualized service plans, and connecting them with appropriate resources.
The Case Manager will also be responsible for monitoring client progress, advocating on behalf of clients, and ensuring compliance with regulations.
The ideal candidate will have excellent communication and problem-solving skills, as well as a strong commitment to helping those in need.
Responsibilities:
Develop and implement individualized service plans for clients.
Provide crisis intervention and support services to clients.
Maintain accurate and up-to-date records of client progress.
Monitor client compliance with treatment plans.
Provide case management services to clients and their families.
Coordinate services with other providers and agencies.
Develop and maintain relationships with community resources.
Conduct assessments of client needs and develop individualized service plans.
